LOINC (Logical Observation Identifiers Names and Codes) is a universal standard maintained by the Regenstrief Institute for identifying clinical observations — primarily laboratory tests but also vital signs, imaging studies, and survey instruments. Each LOINC code identifies a specific observation (e.g. 'serum glucose, mass concentration, point in time, quantitative') along with the units of measure and the result type. The point of LOINC is that two lab systems can exchange a result and both know exactly what was measured, in what units, under what conditions — without bespoke mapping. LOINC is required by HL7 FHIR's Observation resource for lab results, by US Meaningful Use programmes, and by an increasing number of national EHR mandates. EMR vendors integrate LOINC by mapping each lab order in their formulary to the corresponding LOINC code, then transmitting the code alongside the result over HL7 / FHIR.
