ICD (International Classification of Diseases) is the WHO's standard classification of diseases, signs, symptoms, complaints, and procedures. ICD-10 has been the dominant version for the past 30 years; ICD-11 came into effect on 1 January 2022 and is being progressively adopted (the US has signalled a multi-year transition). The classification is used for: clinical coding of encounters (the code-set the doctor or coder assigns to each diagnosis); billing and insurance claims (the codes drive reimbursement); and statistical / epidemiological reporting (the codes aggregate into national disease registries). A modern EMR includes ICD search / autocomplete in the encounter form, often paired with AI suggestions based on the SOAP note text. Vendors deploying outside the US need to support whichever version is mandated in-country, which is increasingly ICD-11 in newer adopters and ICD-10 in legacy systems.
